Initial Appointment

For your initial appointment at North Texas Colon & Rectal, if you are able to download and complete the new patient paperwork, please arrive 15 minutes before your scheduled appointment.

If you need to complete the new patient paperwork in our office immediately before your appointment, please arrive 45 minutes before your scheduled appointment. Alternatively, we can fax or send the paperwork to you if you wish.
